Interview consent form

I ________________________________________________________________ (full name) hereby agree to taking part in a research project about the language used to describe emotional and mental difficulties. I understand that the project will involve being interviewed for short periods on two occasions during my stay in hospital. The interviews will be audiotaped and the tapes will be used only be the researcher. Nothing that I may divulge in the course of the interviews will be passed on to anybody in such a manner as to compromise my right to anonymity. The research will have no bering whatsoever on my treatment.
